Use this form to order new collection boxes or standalone shipping labels for the USO Calling All Patriots recycling program.

  • This field is for validation purposes and should be left unchanged.
  • Standard 12"x12"x12" Collection Box. Each hold approximately 100 cell phones. Each box already has prepaid FedEx return labels attached.
  • Please enter a number from 1 to 15.
  • Order additional FedEx PRP labels for shipping any boxes under 75lbs!
